The reviews consistently praise the book for its remarkable honesty and candor, offering an unvarnished look into the demanding world of neurosurgery. Readers appreciated the author's willingness to admit to mistakes, limitations, and the profound emotional toll of making life-altering decisions daily. The book is lauded for providing a gripping and intensely readable insight into the complexities of the human brain, the intricate details of surgical procedures, and the immense responsibilities surgeons face. Many found the narrative deeply human, exploring the ethical dilemmas surrounding life, death, and the quality of existence, while also highlighting the underlying compassion and care the author holds for his patients. The writing style is frequently described as engaging, clear, and thought-provoking, making complex medical subjects accessible and fascinating.

Despite the widespread acclaim, some reviewers found aspects of the book challenging. A recurring criticism concerns the author's strong personality, which some perceived as arrogant, pompous, or self-important, occasionally leading to a "God complex" or difficult interactions with colleagues and staff. While some found his impassioned critiques of hospital bureaucracy and the National Health Service relatable, others felt these sections were repetitive, extended into lengthy rants, or detracted from the core medical narratives. A few readers also noted that the non-chronological structure of the memoir could be disorienting, and some found the descriptions of surgical details to be quite graphic, potentially off-putting for the squeamish. Additionally, a minority of reviewers felt the book became somewhat repetitive or formulaic in its presentation of patient cases.

Overall, the book is widely recommended as a compelling and profoundly insightful read, offering a unique perspective from the operating theatre. It is particularly suited for anyone interested in medicine, the realities of working within a healthcare system, or the intricate challenges of neurosurgery. Readers who appreciate memoirs that are unflinchingly honest about human fallibility, the stress of high-stakes professions, and the ethical considerations inherent in medical practice will find this book rewarding. It is also highly recommended for those who can look past a strong authorial voice to gain a deeper understanding of the human condition and the extraordinary efforts made by doctors, provided they are not overly sensitive to detailed medical descriptions.
