The "Now" finds a hardened, more cunning Lena, five months after her escape, living undercover in the heart of New York City. She poses as a cured high school student, her days a carefully constructed facade. Her true purpose: to infiltrate the Deliria-Free America (DFA) movement, the very organization that champions the "cure" and demonizes love. Living with Raven and Tack in a small apartment brimming with resistance paraphernalia, Lena has become an active, if still emotionally guarded, member of the rebellion.

A pivotal moment arrives during a massive DFA demonstration. Lena is tasked with keeping an eye on Julian Fineman, the charismatic son of a prominent DFA leader. The event, however, erupts into chaos as Scavengers - a ruthless, destructive faction of uncured individuals - attack, kidnapping Julian. Against all instructions, Lena pursues them, driven by an instinct she cannot fully comprehend, and finds herself captured alongside him.

Trapped in a dark, underground cell, Lena and Julian are forced to rely on each other for survival. He, the poster boy for a world without love, and she, a carrier of the forbidden emotion. In the confined space, amidst the brutality of their captors, an unexpected connection sparks between them. Julian, initially repulsed by Lena's identity as an Invalid, begins to question the truths he has been fed his entire life. He sees the lies, the cruelty, and gradually, a bond of trust, then affection, blossoms between them, challenging Lena's own grief and loyalty to Alex.

Their desperate escape from the Scavengers leads them through the labyrinthine tunnels beneath the city, guided by a group of outcast uncured people. They navigate treacherous paths, their lives intertwined, their burgeoning feelings a dangerous flicker in the oppressive darkness.

Upon their eventual re-capture, Lena discovers a horrifying truth: their kidnapping was no accident. It was a calculated ploy by Raven and Tack, designed to manipulate Julian's public image and ignite further rebellion by orchestrating his execution. Lena, horrified by the cold calculation, resolves to save Julian, even if it means defying her own allies.

She succeeds in rescuing Julian from his impending execution, declaring her love for him in a moment of desperate defiance.
